I would imagine the patients felt a more extreme response to the Co2 because it was their first time feeling fear. If you lived your whole life without fear and suddenly felt it from any source I imagine it would cause a negative feedback loop and you'd feel a stronger response.

Scariest thing I've experienced is nocturnal panic attacks accompanied by sense of impending doom. Happened to me when I was a kid. My appendix burst and I had a surgery. After the surgery I was in ICU and every time I fell asleep, I'd get the attack and had a strong feeling that something really tragic is going to happen and that it was my fault. In the middle of dreaming. Then I'd wake up and think it's reality for the next 20-30 minutes. First time it happened I was waking up after the surgery and it was night, I was alone, and tied to the bed. Later they told me I was tied because I was screaming and wanted to pull out the drainage tubes after the surgery, but I don't remember any of that. Later I found out that it's possible I woke up in the middle of the surgery and that's what caused my panic attacks. But I lost memories of it due to it being a huge trauma. One way or the other, I'd never wish anyone to experience that.

Every humans greatest fear...? Oblivion. Imagine waking up one day and your alone in the universe, floating in some endless black void where you can't see your body, or any lights, stars, or anything. Just a pair of stuck eyes looking around. The place that God did everything He could to escape from, to a point he created a dream world that made Him think hes less alone. If I woke up in such a place, I would rather experience the most terrible horrors like The Thing 1980s and other abominations if it meant that I would feel SOMETHING. To think such a place might actually exist is whats terrifying, it would prove that all of this is just some sad dream of a dead God. That's every creature's fear, not just humans.

The real scariest thing is when you feel something crawling on you in your sleep, but it's your dogs tail.
